Acute Care Team
On Monday 23rd April 2018 we are introducing the services of our Acute Care Team in order to meet the excessive demand for appointments on Mondays.
This service will be operated every Monday until further notice.
Our new Acute Care Team will be made up of
- 2 GPs,
- Nurse Practitioner
- Paramedic Practitioner
When you call or present to the surgery on a Monday you will be offered a telephone call or an appointment with one of our Acute Care Practitioners on the same day.
You will assist both our administrative and clinical teams if you can provide a Reason for your attendance or a description of your illness in order that the most appropriate clinician is assigned to you quickly and effectively.
You may only discuss ONE medical condition during your Acute Care appointment.
Acute Care includes treatment of short-term illnesses, injuries and disorders. If your condition can be managed without the need for a face-to-face consultation, a telephone call will be offered and an Acute Care Practitioner will return your call that day (morning or afternoon).
Can our in-house pharmacist help with your medication?
Our in-house Pharmacist, Abid Pathan is here to help patients with all medication queries, care home repeat prescriptions, hospital discharge medications and all things drug-related. If you have a medication-related problem Abid our pharmacist can help.
Extension of the Home Visiting Paramedics Service
Following the huge success of our pilot scheme, we are pleased to report that our home visiting paramedics scheme is set to continue for the foreseeable future. We have 2 very experienced local paramedics to help the GPs in the 5 surgeries of Woodley Centre, Parkside, Loddon Vale, Twyford and Wargrave with home visits. After visiting and assessing the patient the paramedic phones the surgery duty doctor to discuss the plan of management. Feedback about the service continues to be extremely good, providing early, expert care to housebound patients and saving admissions to hospital and journeys to Accident & Emergency Departments.
